Before you arrive
What to bring to your first appointment
- Any medicines you are taking, including herbal preparations and supplements
- Recent test results or letters from other practitioners, if you have them
- The names of any practitioners already involved in your care
- Any questions you would like the consultancy team to consider
Records are held on paper at the centre. Consent to share your information with named external experts is recorded in writing during your intake consultation.
